ABSTRACT

Objective: To determine maternal and neonatal complications occurring at childbirth among adolescents.Materials and methods: This is a retrospective, descriptive study conducted from 1st July to 31st December 2019 at the maternity ward of the Sylvanus Olympio University Hospital Centre (CHU- SO), Lomé, Togo. The socio- demographic parameters of the mothers, details of prenatal and perinatal events and the clinical profile of the newborns at birth were studied. Results: The records of 332 adolescent mothers were studied. The average age of the mothers was 17.4 ± 1.5 years, with a range of 13-19 years. The pregnancies in two-thirds (66.3%) were supervised in centres without surgical facilities and by midwives in 83.1% of cases. A little over half of the mothers (53.3%) attended at least four antenatal clinic sessions, while 3.6% attended none. The modes of delivery were spontaneous vaginal (62.3%) and Caesarean section (35.2%). Complications of pregnancy were recorded in 12.9% of the mothers. There were statistically significant associations between the referred status of the mothers and haemorrhages, retained placenta and sepsis (p = 0.001, 0.038 and 0.011, respectively). There were no maternal deaths. The newborn babies required resuscitation in 6.3% of cases, while 7.0% were stillborn.There was a statistically significant relationship between the referred status of mothers and the occurrence of perinatal deaths (p =0.0001). Conclusion: Adolescent mothers are at risk of complications during childbirth, and these risks are increased by poor antenatal care and attempted deliveries in centres without surgical facilities.

ABSTRACT

Introduction : L'asphyxie périnatale représente la deuxième cause de mortalité néonatale après la prématurité. Elle est souvent liée dans notre contexte aux circonstances de l'accouchement.Objectifs : Décrire les aspects épidémiologiques, cliniques et pronostiques de l'asphyxie périnatale chez des nouveau-nés à terme à l'unité de néonatologie de l'Hôpital pour Enfants de Diamniadio (Dakar).Patients et méthode : Etude rétrospective, descriptive et analytique menée d'octobre 2014 à novembre 2015. Etaient inclus tous les nouveau-nés à terme hospitalisés dans un contexte d'asphyxie.Résultats : Nous avons colligé 50 cas d'asphyxie, soit une fréquence de 19,5% des nouveau-nés à terme. Le sex-ratio était de 1,5. La majorité des mères soit 80% avait un âge compris entre 18-35 ans. Les pathologies obstétricales rencontrées étaient dominées par la rupture prématurée des membranes 28%, l'infection maternelle 18%, l'HTA 16% et les accidents funiculaires 12%. La naissance par césarienne concernait 1 enfant sur 5. La présentation de siège était la plus fréquente. Le liquide amniotique avait un aspect pathologique (teinté, méconial, purée de pois, hématique) dans 54% des cas. Cinquante-quatre pour cent (54%) des patients avaient un score d'Apgar à 7 à la cinquième minute. L'encéphalopathie anoxo-ischémique, la détresse respiratoire et le choc ont été les complications les plus retrouvées avec 88%, 28% et 22% des cas. Le retentissement biologique était surtout rénal et hépatique dans 24% et 38%. Le phénobarbital était l'anticonvulsivant le plus administré (52%) lors du traitement initial. La mortalité était de 32% (16 patients) ; l'âge moyen de décès était de 36 jours. Le liquide amniotique (p = 0,022), les convulsions (p = 0,041), les troubles de la conscience (p = 0,022), l'EAI au stade 2 et 3 (p = 0,036) et la rupture prématurée des membranes (p = 0,019) étaient significativement associés au risque de décès. Des séquelles étaient observées chez 34,2% des patients : une IMC (11), un trouble du langage (7) et une épilepsie (2).Conclusion : L'asphyxie périnatale reste une pathologie grave. La mortalité élevée et la gravité des séquelles rappellent qu'il est impératif de renforcer la prévention par un bon suivi des grossesses et de l'accouchement

Introduction: Dans les pays en développement l'asphyxie périnatale constitue l'une des principales causes de décès néonataux. La plupart des facteurs de risque associés peuvent être prévenus. L'objectif de cette étude était d'étudier les facteurs de risque associés à l'asphyxie périnatale, et le pronostic des nouveau-nés ayant présenté une asphyxie dans l'unité de néonatologie du CNHU-HKM de Cotonou.Patients et méthodes : Il s'agissait d'une étude cas-témoin réalisée du 1er avril au 31 août 2015. Les données de 64 nouveau-nés à terme ayant présenté une asphyxie ont été comparées à celles de 128 nouveau-nés à terme sans asphyxie. Résultats : La prévalence était de 4,5%. Les facteurs associés à l'asphyxie étaient la présence d'une affection au cours de la grossesse (p= 0,004 OR 7,05 [1,84 ­ 26,96]), le long travail d'accouchement(p= 0,041 OR 3,09 [1,04 ­ 9,16]), la rupture prématurée des membranes (p=0,002 OR 17,51[2,93 104,56]), le liquide amniotique teinté et/ou fétide (p<0,001 OR 137,18 [29,18 - 651,03]). L'évaluation des nouveau-nés les 24 premières heures avait noté une encéphalopathie anoxo-ischémique : grade 1 (36%) ; grade 2 (45%) ; grade 3 (19%) selon la classification de Sarnat. A la 48ème heure une encéphalopathie anoxo-ischémique était présente chez 87,5% des nouveau-nés (n= 56). Elle était de grade 2 et de grade 3 dans 46,3% des cas. Le taux de décès des nouveau-nés étaient de 26,6%. Conclusion : Le dépistage systématique des facteurs de risque avec mesures correctrices conséquentes permettraient d'optimiser la prise en charge des nouveau-nés

ABSTRACT

Background: The anthropometric parameters of the umbilical cord have clinical significance. Current parameters of the cord, its correlates and related foetal outcome are lacking in our parturients. Objectives: To describe the anthropometric parameters and abnormalities of the umbilical cord; and determine their maternal correlates and foetal outcome. Design: A cross sectional analytical study. Setting: The Obstetric and Gynaecology Department of the University of Ilorin Teaching Hospital, between September 2012 and June 2013. Subjects: Healthy pregnant women with singleton pregnancies. Results: Four hundred and twenty- eight (428) singleton deliveries were studied. The respective mean values of the cord length and width were 526.87±115.5mm and 19.56±11.12mm. Short cord (< 40cm) occurred in 7.2% while long cord (> 69cm) was found in 9.3% of the parturient. The incidences of single umbilical artery, cord round the body and knots were 7%, 8.4% and 14.5% respectively. Nuchal cord was the most common (91.4%). Only gestational age had significant statistical relationship with cord length abnormalities (P = 0.0093). The cord length was an important correlate of cord helices, knots and vessels (P< 0.05).Parity had correlations with the number of vessels(R= 0.099, P=0.042). The cord coiling index was statistically related to the presence of congenital abnormalities (P=0.011). Other perinatal events were not related to umbilical cord parameters. Perinatal asphyxia was the most common indication for NICU admission (3.5%) but there was no significant statistical difference between NICU admission and cord parameters. Conclusion: The umbilical cord parameters in apparently healthy parturients in Ilorin were comparable with others elsewhere. The cord length and helix are important correlates of gestational age and congenital abnormalities. Parity may be related to abnormal umbilical vessels. Cord length, coils, coil index and umbilical vessels should be examined post-natally

Background: Health education is an important tool required for reducing the burden of birth asphyxia in the developing world. Objective: The objective of this study was to assess the knowledge of mothers; who received health facility-based antenatal care during their last pregnancy; about birth asphyxia and relate their knowledge to their places of antenatal care. Materials and Methods: A cross-sectional survey of mothers of infants attending the Immunization clinic in a Nigerian Teaching Hospital was done between July and October 2010 using a close-ended questionnaire. Consecutively consenting mothers were enrolled into the study. Results: Out of 354 mothers; 56.5 received counseling about birth asphyxia during antenatal clinic visits in their last pregnancy; 85.5 of attendees of teaching hospital; and 26.4 of attendees of private antenatal clinics received counseling about birth asphyxia. Overall; 38.9 of the respondents had satisfactory knowledge about birth asphyxia; 47.5 of teaching hospital attendees; and 28.1 of private clinic attendees had satisfactory knowledge about birth asphyxia. Lower socioeconomic status; lack of counseling; and nonattendance of teaching hospital antenatal clinic were associated with poor knowledge about birth asphyxia. Conclusion: Most of the mothers surveyed had poor knowledge about the risk factors and sequelae of birth asphyxia. The health system needs to improve health education of expectant mothers about birth asphyxia

ABSTRACT

Introduction: Competence in neonatal resuscitation; which represents the most urgent pediatric clinical situation; is critical in delivery rooms to ensure safety and health of newly born infants. The challenges experienced by health care providers during this procedure are unique due to different causes of cardio respiratory arrest. This study aimed at assessing the knowledge of health providers on neonatal resuscitation. Methods: Data were gathered among 192 health providers drawn from all counties of Kenya. The clinicians were asked to complete questionnaires which were in two parts as; demographic information and assessment of their knowledge by different scenarios which were formatted in the multiple choice questions. Data were analyzed using SPSS version 15.0 for windows. The results are presented using tables: Results: All the participants were aged 23 years and above with at least a certificate training. Most medical providers had heard of neonatal resuscitation (85.4) with only 23 receiving formal training. The average duration of neonatal training was 3 hours with 50 having missed out on practical exposure. When asked on steps of resuscitation; only 68 (35.4) of the participants scored above 85. More than 70 of them considered their knowledge about neonatal resuscitation inadequate and blamed it on inadequate medical training programs. Conclusion: Health providers; as the key personnel in the management of neonatal resuscitation; in this survey seem to have inadequate training and knowledge on this subject. Increasing the duration and quality of formal training should be considered during the pre-service medical education to ensure acceptable neonatal outcome

Notre etude; en vue de contribuer a l'amelioration de la prise en charge des souffrances foetales aigues du nouveau ne a terme a retenu les facteurs de risque les plus lies a l'asphyxie perinatale : -L'age de la mere inferieur ou egal a 15 ans ; -La qualite de la consultation prenatale avec le bilan prenatal biologique non fait (39;2pour cent) ou incomplet (54;9pour cent) ; -Le lieu de naissance; en l'occurrence la maternite. (21;8pour cent) en rapport avec les mauvaises conditions socio economiques ; -Le sexe masculin du nouveau ne (sexe ratio de 1; 91). Sur le plan clinique; le diagnostic est retenu avec: -Le score d'APGAR 7 a la cinquieme minute (94;1pour cent) ; -Un tableau neurologique fait de perturbations du tonus (62;7pour cent) et des reflexes archaiques (100pour cent) ainsi que de convulsions. (16;7pour cent). Il lui est associe le plus souvent une infection neonatale (81;37pour cent) et le traumatisme obstetrical (6;86pour cent). Au niveau du volet exploration radiologique: L'echographie transfontanellaire a retrouve (10;8pour cent) de lesions avec 5 hydrocephalies; 5 oedemes cerebraux et une hemorragie du noyau caude. La prise en charge symptomatique et preventive a ete faite par : -Un apport hydroelectrolytique a minima; -Une cortieotherapie; -L'oxygenotherapie; -Des anti convulsivants; -Une antibiotherapie en cas d'infection patente ou probable; -Une alimentation par gavage (61pour cent a J3) puis la mise au sein des que possible. La rarete des examens paracliniques de diagnostic et de surveillance est liee aux conditions socio economiques defavorables et au plateau technique limite. La guerison s'obtient pour la majorite en 6 jours
